Hospice Licensed Practical Nurse (LPN)
Kansas City Metro, MO (east) | Full-Time | Monday-Friday
Outstanding hospice provider seeks a full-time Hospice LPN. This nurse will provide skilled, compassionate care to hospice patients, support symptom management, monitor changes in condition, educate patients and families, and work closely with the interdisciplinary care team.
Required Qualifications
· Graduate of an accredited Licensed Practical Nurse program
· Current LPN license valid for practice in Missouri
· Current driver's license
· Minimum of one year of nursing experience
· Hospice or palliative care experience preferred
· Reliable transportation and valid automobile liability insurance
· Ability to work independently and make sound clinical decisions
· Strong written, verbal, organizational, and interpersonal skills
Key Responsibilities
· Provide direct nursing care to hospice patients in home and facility settings
· Assess patient needs and monitor symptoms, vital signs, and changes in condition
· Administer medications, treatments, and interventions as ordered
· Educate and support patients and families regarding symptom management and end-of-life care
· Communicate changes and concerns promptly to the RN and interdisciplinary care team
· Complete all required documentation accurately and on time
