Hospice Admissions RN
Kansas City Metro, MO (east) | Full-Time
Outstanding hospice provider seeks a full-time Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.
Required Qualifications
· Current Registered Nurse license valid for practice in Missouri
· Current driver's license
· Current CPR certification
· Two years of Registered Nurse experience in a clinical care setting
· One year of full-time hospice experience
· Reliable transportation and valid automobile liability insurance
· Ability to work independently and make sound clinical decisions
· Strong written, verbal, organizational, and interpersonal skills
Key Responsibilities
· Coordinate and oversee all aspects of new patient admissions
· Collaborate with patient care teams to support seamless communication and continuity of care
· Educate patients and families regarding condition, needs, and care
· Perform head-to-toe physical assessments
· Complete all required documentation accurately and on time
· Perform other duties as assigned
