Social Worker - Occupational Health
A growing occupational health clinic located at a satellite site in Albany, NY is seeking a Social Worker to join its clinical team. This role supports patients navigating workplace injury, return to work planning, and connection to community and behavioral health resources.
Responsibilities:
• Conduct psychosocial assessments for patients seen through occupational health services
• Support return to work planning in coordination with providers, employers, and case managers
• Connect patients to community resources, behavioral health services, and financial or social supports as needed
• Assist with care coordination for complex or high risk occupational injury cases
• Document assessments and interventions in the EMR in line with clinic and regulatory standards
• Serve as a liaison between patients, providers, and employer or insurance contacts when psychosocial factors affect recovery or return to work
Qualifications:
• Master's degree in Social Work (MSW) required; LCSW preferred - LMSW considered.
• Experience in occupational health, workers compensation, disability, or a related medical setting preferred
• Strong assessment and care coordination skills
• Comfortable collaborating with medical providers and employer partners
• Excellent communication and documentation skills
Compensation:
• Market rate in the Albany, NY area runs approximately $65,000 to $95,000 per year ($31.25 to $45.67 per hour, depending on experience), with LCSW licensure and occupational or medical setting experience trending toward the higher end.
Schedule and Location:
• Full time or part time, on site at a satellite location in Albany, NY, depending on program needs.
