Hospice RN Case Manager
Kansas City Metro, MO (east) | Full-Time
Outstanding hospice provider seeks a full-time RN Case Manager. This nurse will coordinate and oversee patient care, develop and implement individualized plans of care, provide clinical assessment and symptom management, and work closely with the interdisciplinary team to support patients and families.
Required Qualifications
· Current Registered Nurse license valid for practice in Missouri
· Current driver's license
· Current CPR certification
· Two years of Registered Nurse experience in a clinical care setting
· One year of full-time hospice experience preferred
· Reliable transportation and valid automobile liability insurance
· Ability to work independently and make sound clinical decisions
· Strong written, verbal, organizational, and interpersonal skills
Key Responsibilities
· Coordinate and oversee all aspects of assigned patients' hospice care
· Develop and implement individualized plans of care with the interdisciplinary team
· Perform head-to-toe assessments, monitor symptoms, and manage medications
· Educate patients and families regarding condition, needs, and care
· Communicate patient progress with clinical leadership, physicians, and the care team
· Complete all required documentation accurately and on time
